2. You are given this slip because you have travelled from China.
3. Please keep this slip with you and watch your health for 14 days.
4. If you develop fever, cough, or have difficulty breathing during this time, seek medical care right away.
5. Call ahead before going to a doctor, clinic or hospital.
6. Tell them of your travel history and what symptoms you have.
7. Give this slip to the doctor.
